A risk assessment is a written record of the hazards present in your work, who could be harmed, and the steps you are taking to reduce that harm to an acceptable level.
A useful assessment describes the activity as it is actually carried out, names the controls already in place, and identifies any further action with an owner and a date. Assessments written in generic language rarely change behaviour on the ground.
Responsibility sits with the employer. It can be prepared with support from a consultant, but the arrangements it describes must reflect your workplace.
